Getting Braces Put On

The appointment itself is not painful. Placing brackets takes about 60 to 90 minutes. There's nothing that causes sharp pain during the process — the most uncomfortable part is keeping your mouth open for an extended period. Some patients find the cheek retractors mildly uncomfortable, but it fades quickly.

The first 24 to 72 hours after brackets are placed is when you feel the treatment. The teeth begin to move, and this causes a dull aching pressure — similar to the feeling of a bruise being pressed, but in your jaw. It's not sharp pain, but it's real, and it peaks around day two.

Soft foods during this window help significantly. Yogurt, mashed potatoes, scrambled eggs, soup, pasta. Most patients feel close to normal by day four or five.

Adjustment Appointments

Every 6 to 8 weeks, the wire is changed or tightened to continue moving teeth. This is quick — 15 to 20 minutes — and the process itself is mild. The 24 to 48 hours following an adjustment often bring the same dull pressure as the initial placement, though usually less intense because your teeth are already partly adapted to the feeling.

Ibuprofen (if you have no contraindications) taken an hour before an adjustment appointment and for the day following is a strategy many patients find helpful.

Invisalign Discomfort

Clear aligners work differently but produce a similar experience. The first tray and each new tray thereafter causes pressure for the first 24 to 48 hours — the sensation of the aligner pushing teeth in a direction they haven't gone yet.

Most patients describe aligner discomfort as slightly milder than braces adjustments, and it tends to become progressively less noticeable as treatment progresses and teeth are closer to their final positions.

The other notable discomfort with aligners: a small lisp is common in the first week and goes away on its own.

Soft Tissue Irritation

Braces involve brackets and wires, which occasionally rub against the inside of the cheeks or lips. This is most common at the beginning of treatment before the soft tissue toughens up. Orthodontic wax (given to you at the clinic) pressed over any offending bracket solves this instantly. A protruding wire end can usually be bent back with a pencil eraser — and if not, the clinic can fix it at a quick appointment.

Aligners have smooth edges and generally cause no soft tissue irritation.

What's Worth Calling the Clinic About

Dull aching and pressure after placement or adjustment: completely normal, manage with soft diet and over-the-counter pain relief. A bracket that comes off: not a pain emergency, but call the clinic to schedule a repair — don't wait until your next regular appointment, especially if the bracket is on a front tooth. A wire that's poking and can't be managed with wax: call for a same-day fix, most clinics fit these in quickly. Sharp, severe, or persistent pain in a specific tooth: this can occasionally indicate an issue that needs attention, and it's worth calling to describe what you're experiencing. Significant swelling or swelling that spreads: see a dentist or urgent care promptly.
